Neck Lift
You can choose a Plastic Surgeon for Location, or results!BENEFITS
Awareness of a sagging neck can first surface as a dislike of being photographed in profile. The profile view unforgivingly show the hanging skin and muscle that are at the heart of the issue. The neck can look like a a gradual slope from the chin to the Adam’s Apple from this angle. A loss of definition along the chin will often accompany the hanging tissue. The Minimal Incision Necklift delivers a revitalized look with smaller incisions and a reasonable recovery. You also have the benefit of local anesthesia performed in our out-patient suite as opposed to a stay at the hospital.
DESCRIPTION
To correct the aging neck, a healthy improvement can be made through the surgical removal of skin along with the tightening of the underlying muscle. Further benefit can be achieved with liposuction of any fat deposits that are aggravating the situation. An incision is made underneath the chin to access the muscles and fat.
RISKS
All surgical procedures carry risk. Anyone undergoing surgery must understand that even though the odds of a major event are small, risks from surgery are real. Whenever the skin is broken, there is the possibility of infection, necrosis, swelling, bruising, scarring, discoloration, nerve damage, itching, hematoma, pain, etc. Our goal is safety. We use a local anesthetic to decrease the risks associated with general anesthesia. We have you back for several post-op visits to monitor your healing. We are open 7 days each week and are accessible via email. We work together to reduce risk.
Out of our last 100 cases this year, patient surveys revealed only 11% of our patients had bruising
after cosmetic facial surgery and 98% used no narcotics after their cosmetic facial surgery!
We always recommend Celebrex, which is an anti-inflammatory postoperatively for
anywhere from 5 to 7 days depending on what surgery the patient is getting.

NECK LIFT BEFORE AND AFTER PICTURES
74-year-old female before and 6 months after a mini neck lift. Patient was happy with the mild improvement of her neck. She had a very small scar in her submental crease. This was a 14 minute procedure done in the office under local anesthesia. The mini neck lift scar measured only 3 cm.
74-year-old female before and 6 months after a mini neck lift. It is very important not just to look at static pictures. Patients look very different when they are looking down, animating or moving around. In this picture, you will notice the patient is looking downwards and trying to touch her chin to her chest. This actually reveals a much more dramatic result from her mini neck lift then you would expect to see, especially because you're not really cutting out much skin with such a small scar.
62 year old male before and 6 months after a full scar neck lift. Patient felt a lot more confident without the excess skin and fat in his neck. A lot of people are bothered by turkey neck and a full neck lift is the most dramatic improvement we can offer them.
62 year old male before and 6 months after a full scar neck lift. Notice the dramatic improvement with the patient looking downward. These lateral views are where you can most appreciate the amount of skin and fat removed.
78-year-old female before and 2 months after a mini scar lateral neck lift. You'll notice the dramatic improvement without any scarring in the midline. Patient had enough skin laxity to warrant a full scar neck lift but did not want the anterior scar. So the scar she got with this new minimally invasive surgery was a 3 cm lateral neck scar tucked under her jaw line and a 1 cm scar in the hair.
78-year-old female before and 2 months after a mini scar lateral neck lift. Notice how the submental angle is now harmonious in the lateral view. This is not a thread lift which does not give much improvement and does not last. After 1 year most thread lift patients notice only a 1-2 mm difference. Here, the SMAS muscles are tightened with 2-0 PDS internally which absorbs after four months and these are the same sutures we use for facelifts and full neck lifts. The result is much more dramatic.
78-year-old female before and 2 months after a mini scar lateral neck lift. Notice this view from underneath to fully show the scar. From normal viewing, the scar is not obvious. Of course the scar is red because not enough time has not passed for scar maturation which can take 6 to 12 months. Patient is ecstatic with her results.
87 year old female patient before,6 weeks and 3 months after direct neck lift. Notice the patient had no other surgery. Notice the remarkable difference in the neck contour. There is still some puckering which usually disappears within 1-3 months after the procedure. Patients who are willing to tolerate these scars and do not want an extensive procedure are excellent candidates for this operation. Remember, this was a quick procedure done with minimal sedation and local anesthesia that took about 30 minutes!
87 year old female patient before,6 weeks and 6 months after direct neck lift. The patient had no other surgery. Notice the improvement of her "turkey gobbler" neck.
87 year old female patient before,6 weeks and 6 months after direct neck lift. The patient had no other surgery. Notice the sustained improvement even with neck flexion
87 year old female patient before, 6 weeks and 6 months after direct neck lift. The patient had no other surgery. Notice the dramatic improvement even with neck flexion.
A 63 year old female patient before and 6 years after a direct mini T or Z neck lift done in the office under local anesthesia which took only 21 minutes. She didn’t want any other cosmetic facial surgery.
67 year old Hispanic female patient before and 1 year after a Short scar facelift, midface lift, neck lift and upper and lower blepharoplasty. Notice the dramatic improvement in neck skin laxity. There were no other procedures performed.
57 year old female patient before and 6 months after a Short scar facelift, midface lift and upper and lower blepharoplasty. Notice the natural appearance achieved with minimal scarring and down-time.
54 year old female before and 1 day after mini facelift and mini neck lift(small submental scar). Two extra 1 centimeter scars allowed a more powerful lifting procedure to further improve her neck laterally. A thread lift cannot come close to any result like this. The entire lateral neck area was released and freed endoscopically (similar to what a ponytail facelift does higher). She is ecstatic with the results.
A 65 year old male patient before and 6 months after a direct necklift done in the office under local anesthesia which took only 20 minutes. Even though he was a candidate to get other procedures such as a facelift, cosmetic eyelid surgery, as well as injectables, the only thing that bothered him with his neck and he was willing to tolerate a small T scar. This is a very gratifying operation especially for men who can hide the scar in their beard. His retruded chin was also elevated (pushed upwards) with the procedure to look more normal and even his jowls were improved.
Same patient - notice the scars blending in without the use of any makeup or concealer. The hardest part to this operation is the initial postoperative period and looking at the red thick scar but as with all scars, it usually softens within 6-12 months to become a soft white line in most patients.
Same patient - notice the natural and dramatic difference.
54 year old female before and 1 day after mini facelift and mini neck lift (small submental scar). Patient also did not mind having an additional two scars - one in a lateral neck ring and one behind the ear in her hairline. These two additional small 1 centimeter incisions allowed a more powerful lifting procedure to further improve her neck. Her neck had a lot of skin laxity and she really needed a full neck lift scar to properly address this. We call this a modified mini lateral neck lift and this procedure has not been described by anyone previously.
A 45 year old female patient before and 1 day after a mini necklift done in the office under local anesthesia which took only 10 minutes. She is very happy with her results and had no additional procedures done. As with most of our patients, she was surprised how quick the procedure was and that she needed to take no pain medicine after surgery.
66 year old male patient before and 5 months after direct neck lift. Patient did not want any other surgery. Notice the remarkable difference in the neck contour with downward flexion. This surgery was done in the office with only local anesthesia that took 42 minutes.
64 year old female patient before, 6 months and 11 years after Mini Face Lift and Mini Neck Lift. Patient did not want a lot of scars and was scared of general anesthesia. Both surgeries were done in the office in 41 minutes with only local anesthesia.
The Chicago Neck Lift Plastic Surgery Scars:
Anytime a human being gets cut (with surgery or trauma), scars heal the following way: Initially they are red, raised and hard. Within 6-12 months, they are flat, white and soft. Younger people (<18 years old) have scars that might stay active or red for up to 2 years. Older people (>65 years old) have scars that might stay active or red for less than 3 months
74-year-old female 6 months after a mini neck lift. Notice how small the scar is in length and it is in the submental crease. You can see from all angles that it is not obvious. Patient was very happy with her mini or short scar neck lift results and her scar placement. The patient started using Dr. Speron’s Natural Skin Care Natural Scar Support at her 2 week postoperative appointment and has continued using it twice a day.
45 year old female 1 day after a mini neck lift. Noticed there is no evidence of bruising and even with the swelling the patient was happy with her results. This procedure took 14 minutes in the office under local anesthesia.
48-year-old female 1 day after only a lateral mini neck lift. This procedure only has two small 1 cm scars, one on the lateral neck, one behind the ear. Notice how the puckers are most prominent right after surgery especially because no skin is removed. The inside PDS stitch lifts the Smas/Platysma muscle and tightens the sling while absorbing over 4 months. This is not a thread lift which does not show much permanent difference after the first week. These puckers should be massaged and usually flatten very quickly.
A 77 year old female patient before and 2 weeks after a full scar neck lift done in the office under local anesthesia which took only 29 minutes. Notice the puckering of the scars which will flatten over time. The redness and firmness of the scar will fade to a thin flat white scar usually within 6-12 months.
A 59 year old female patient 6 months after a full scar neck lift done in the office under local anesthesia. The scar has matured into a very thin flat white scar. Notice in the second picture that the thin scar falls inside of this red outline. The scar under the neck is a T or Z depending on the patient's preferences, anatomy and skin excess. The reason we do not do a vertical scar in the submental area is because then the patient has restriction when they extend their neck backwards. This is discussed with the patient and reviewed with a mirror before surgery.
There are four different options to improve a neck. Depending on how much extra skin you have, fat, and how lax your muscles are these options will be discussed during your private consultation. First option is just liposuction. Second option is a mini neck lift through a small scar in the submental crease. This pulls the neck medially or inward. Next option is a mini lateral neck lift, which has two small scars, and pulls the neck laterally or out to the side. Finally, the last two options are a T or Z neck lift, where the submental skin excess is removed and a full neck lift, which includes the T or Z scar as well as a vertical scar all the way down to the sternal notch. This last option is the most aggressive, but has the most scarring.
In a lower blepharoplasty, typically a canthopexy is done at the same time to tighten the loose orbicularis muscle and give a more natural look while diminishing ectropion or lateral scleral show.
A Midface or cheek lift is done through the same incision as a lower blepharoplasty with the addition of a small incision in the hairline. The muscle is lifted in two directions, one with the lower blepharoplasty and canthopexy, and secondly, in a different vector superiorly to lift the mid face or cheek. Most people lift this laterally, but I have changed my approach, because that did not look as natural and it was not as powerful a lift. The midface or cheek ages and descends downward. Please remember that with a mid face or cheek lift, you are also getting a lower blepharoplasty at the same time. It is included and not a separate procedure.
Most surgeons will lift the muscle or SMAS in 1 direction. I have evolved my technique over the years to lift it in 3 separate directions, which results in much more powerful lift, as well as a more natural look. The 3 vectors are determined the day of surgery, depending on what priorities the patient has in terms of jowls, jawline, neck etc. Even though this improves the midface about 30% and the neck, further work can be done with a midface or cheek lift, as well as additional work to the neck, depending on the patient's examination and their expectations.
More Neck Lift Surgery Before & After Pictures:
87 year old female 3 months after full scar direct neck lift. There is still some puckering which usually disappears within 1-3 months after the procedure. With a full scar neck lift, the vertical scar can be seen but the submental scars typically are not exposed unless a patient is looking backward and fully extending the neck.
A 65 year old male 6 months after a direct necklift. His scars are still a bit red and we suggested he continue using the scar product and massaging aggressively. There's not much we would recommend for laser treatments here, but we would normally recommend everywhere else. There's a little overlap with laser hair removal and we do not want to promote and remove any hair because the beard hair camouflages the scar. So within a man's beard is the one area we don't typically recommend postoperative laser scar treatments.
67 year old Hispanic female 1 year after a Short scar facelift, midface lift, mini neck lift and upper and lower blepharoplasty. Notice the mini neck lift scar is tucked naturally and imperceptible.
62 year old male 6 months after a full scar neck lift. He was very comfortable with the scar compared to what he had before surgery. Obviously most men can camouflage most of the scar in their beard line. The scar can be tailored and be longer or shorter depending on what excess skin bothers the patient. It can even be fully within the beard line. Once patient's scars are mature and they are happy with their size and orientation, they can get FUE (minimally invasive hair transplants without cutting and big occipital scars) into the mature white surgical scars to further camouflage them. For areas outside the beard line, they can also consider getting cosmetic tattooing to make the white scar more flesh colored and thus less noticeable.
68 year old female before and 2 months after mini lateral neck lift (2 small 1 cm incisions). Her neck had a small area of skin laxity that bothered her. She needs a full neck lift because she has a lot of extra skin but was happy with this tiny scar approach.
65 year old female patient before and 4 years after a direct necklift done in the office under local anesthesia which took only 32 minutes. This patient wanted the incision extended past the sternal notch into the upper part of the chest to remove that other old scar from previous CV surgery.
65 year old female patient before and 4 years after a direct necklift done in the office under local anesthesia. Noticed the dramatic improvement because of the large amount of skin removed. Notice also that not all of her folds were able to be removed because she still has a large amount of excess skin laterally. She is very happy with the results and is not interested in a lateral neck lift.
